Client Forms
If you are a new client and have contacted me to set up a consultation meeting please print out and complete the New Client Forms which includes Pet Profile, Veterinarian Release and Pet Care/Client Profile and Contract. A Pet Profile is needed for all pets. Please print out the forms and complete all applicable sections. I recommend you make a copy of the completed forms for your records, but it is not required. I will need the original completed forms ready at the consultation meeting so I can review them with you and keep in your file. At the meeting I will go over any specific details about your service not covered in the contract. New Client forms:
If you would like your dog to be taken to a dog park or hiking trail for off-leash exercise please print and complete the Authorization and Waiver Document. Please be sure to sign in all three required places. You can mail the authorization and waiver document to the address on the form, or leave it at your house to be picked up on the first visit.
